Insurance
I am not accepting insurance at this time but can provide you with a statement you can submit to your insurance for reimbursement.

Payment
Payment is due at the time services are rendered. Cash and checks, Zelle, Venmo, PayPal and Apple Pay are accepted forms of payment.

Cancellation Policy
If you do not show up for your scheduled appointment, you will be required to pay the full cost of the treatment as booked.  If you call 24 hours in advance to cancel or reschedule your appointment, you will not be charged.
